    The Bill Melinda Gates Foundation intends to become the key investor in improvements to sanitation in developing countries with the announcement Tuesday of a $42 million grant scheme to spur innovation and encourage behavioral changes.     While child mortality is on the decline worldwide, an estimated 40 million children under five in poor countries remain at high risk of death because they do not have access to basic vaccines, such as diphtheria and tetanus, or oral rehydration therapy for diarrhea, according to a report by Save the Children.
